Revised Fee Schedule
On-Line Verification | Disciplinary Actions | Revised Fee Schedule | Primary Source Verification

The Texas State Board of Podiatric Medical Examiners has/had experienced a substantial increase in the volume of license verification requests received by our agency’s staff, both via telephone and in writing. As a result of the negative impact that immediate servicing of multiple license requestors and budget reductions have had on our ability to address other agency business and telephone calls, the following revised policy is effective as of September 4, 2001 [Updated February 17, 2012]:

  1. On-Line License Verifications:

There is NO FEE for On-line verifications.

  2. Telephonic License Verification Inquiries:

No more than Three (3) license verifications Per Day will be provided to any one entity via telephone, (512)-305-7000.

  3. Written License Verification & License Certification Requests:

Payment must accompany requests for “Verifications” and “Certifications,” or the Request will not be honored. A stamped, self-addressed envelope must also be enclosed. Exact payment must be submitted, overpayments will not be treated as credit for future requests.

    1. License Verifications (written, on any form) ... $25.00 per name (Intended for hospitals, insurance companies, etc.)

    2. License Certifications (on any form) ... $75.00 (Intended for state licensing boards requesting proof of licensure.)

  4. Database List Orders:
  5. If the above verification policy does not appropriately address a requesting entity’s needs, they may obtain the verification information they require in the following formats:

    1. CD/Electronic Mail of all registered podiatric physicians (statewide) ... $300.00

    2. Printed List/Labels of all registered podiatric physicians (statewide) ... $400.00

    3. Breakdown of any one city/county/zip code/etc., provided Electronically or via Printed List/Labels (for each individual breakdown) ... $100.00

    4. Click here to download the DATABASE LIST ORDER FORM.

    5. To conduct a (limited) FREE database search, please take note of the following. This basic "self-serve" alternative method requires time and mining capabilities of  your own. A) Visit the Board's "License Verifications" page. B) Click on "View Physician Profile." C) Once at the Texas Online "Professional Profiling System" page, for purposes of this basic "self-serve" alternative method, you may conduct a search for any Texas licensed "Podiatrist" by City, State or Zip Code. (Searching by Zip Codes allows for a broader return of results by geographic area.) D) The actual license "Status" can be verified by searching the podiatrist's name or license number in the first field on the "License Verifications" page. E) It will then be up to you to determine how to collect and transfer the search results to your system. This will require patience and effort on your part. The Board makes this alternative method available as some requestors have financial constraints to paying the fees listed above.

  6. Miscellaneous Charges:

Electronic verifications for Database List Orders are $1.00 per name/record (the number of records are estimated for the expended total number of individuals licensed per the agency's operating budget for the recently completed fiscal year), plus postage & calculation of staff labor/time/overhead costs.

Long Distance Facsimiles ... $1.00 per page

Copies (for Open Records requests, etc.) ... $0.10 per page, plus postage & calculation of staff labor/time/overhead costs.

If you have any questions or comments, please contact agency staff at 512-305-7000. Agency staff, at their sole discretion, for good cause, may grant one-time exceptions to the fee schedule requirements. This schedule has been revised in the wake of the 82nd Texas Legislative Session to reflect the increase in the cost of doing business and demand on agency resources ... to include budget reductions.

The fees set and collected by the Board must be sufficient to meet the expenses of administering the Podiatric Medical Practice Act of Texas and the applicable Board Rules/Regulations.
