Primary Source Verification
On-Line Verification | Disciplinary Actions | Revised Fee Schedule | Primary Source Verification

One of the NCQA requirements is primary source verification of medical education, which include:

  1. Identity and age

  2. Certified transcripts

  3. Copy of Medical School Diploma

  4. Professional training and ability

  5. Application to Dean with school seal

  6. Other state licenses verified

All of the aforementioned (A-F) are obtained as part of the Podiatric Licensure process. In addition, the following is a complete list of requirements for a permanent license:

  1. Transcripts from ALL colleges attended (undergrad plus podiatry)

  2. Passing National Board Scores (Part 1 and Part 2)

  3. Passing PM Lexis Scores (overall score of 75 or above)

  4. Certification letters from all states in which a license is held

  5. Copy of podiatry diploma

  6. Current CPR training (basic or ACLS)

  7. Current photo

  8. Dean’s signature on front of application (suggest that after the application is filled out, they make a copy and send the copy to the Dean)

  9. Self-Query Report from the National Practitioner Data Bank

  10. Completion of at least 1 year of an APMA approved residency

  11. $289.00 Application Fee

  12. Two (2) FBI finger print cards (effective with the October 14, 2005 examination)
