One of the
NCQA requirements is primary source verification of medical education, which
include:

Identity and age
-
Certified transcripts
-
Copy of Medical School Diploma

Professional training and ability
-
Application to Dean with school seal
-
Other state licenses verified
All of the aforementioned (A-F) are obtained as part of the
Podiatric
Licensure process. In addition, the following is a complete list of
requirements for a permanent license:

Transcripts from ALL colleges attended (undergrad plus podiatry)

Passing National Board Scores (Part 1 and Part 2)

Passing PM Lexis Scores (overall score of 75 or above)
-
Certification letters from all states in which a license is held
-
Copy of podiatry diploma
-
Current CPR training (basic or ACLS)
-
Current photo
-
Dean’s signature on front of application (suggest that after the application
is filled out, they make a copy and send the copy to the Dean)

Self-Query Report from the National Practitioner Data Bank
-
Completion of at least 1 year of an APMA approved residency
-

Two (2) FBI finger print cards (effective
with the October 14, 2005 examination)
